Module 6 Introduction
This module highlights the importance of service coordination in the case management process. Because it is the case manager’s responsibility to locate resources, make arrangements for clients to use them, and monitor client use and progress, coordinating services is a critical phase of case management. A case manager establishes a network of professional colleagues representing a variety of agencies and organizations to support the case management process and best meet clients’ needs. Case managers work within both a collaborative network and their own agency’s organizational context and structure when coordinating services and making referrals. As a result, it is important for case managers to recognize the formal and informal organizational structures that exist in agencies.
During service coordination, the case manager and the client agree on a plan of service and then determine what services can be provided by the case manager and what services need to be referred. It is vital for the client to participate during resource selection so that his or her values and preferences and unique circumstances are considered. Readiness of the client to be referred, appropriateness of the referral, and willingness of the new agency to receive the client are all important factors in making a successful referral.
