The Field Journal is designed as a place for you to process your cognitive and affective responses to every aspect of your practicum experience, including direct service with clients/constituents, supervisory and collegial relationships, organizational policies and politics, and your relationship with yourself as a growing professional. Since you will be submitting your Field Journal to your Course Instructor in Weeks 2 through 8, you may be tempted to censor yourself when you write in it. While you are strongly encourage to be transparent so that your Course Instructor can help you work through any issues that come up in meeting your learning objectives, it is understandable that you may want to create one document as a publish in which you can be as messy as you like. You can put down your uncensored thoughts and feelings without having to worry about how they come across. Later, you can copy, paste, and edit your entries into a final publish.
Guidelines
Please download the form provided and complete it for as many days as you work at your practicum site(9/26-9/30). If you miss a day due to illness or other events, make a brief entry to that effect. The amount of space given is expandable and is not meant to represent an ideal length. See the sample entry below the blank form. Remember to use initials for clients to protect their privacy.
What to discuss:
Day 1- Training
Dat 2- group supervision/task assignment(SBIRT)
Day3- Individual supervision discussed assignment(SBIRT)
Day4- meeting with the program director to discuss assignment(SBIRT)
Day 5- research assignment(SBIRT)
