I need to do a reflection paper on my nursing clinical. It was done at Little company of mary hospital in evergreen park illinois. My project was to improve documentation on blood administration. The problem was things were being missed or left out of the charting and as a result, the hospital wasnt getting reimbursed. I reviewed Joint Commision standards on documentation. I did chart audits to see what was being missed and to look for any patterns of error. I surveyed nurses as to why this was happening, and to get their preferred method of learning. I attended a quality improvement meeting to get ideas for my project. Then I came up with a checklist for the nurses to use each time they document a blood transfussion.
