The goal in staffing models is to minimize cost while staying effective in treatment for patients that we care for. In healthcare, nurses are the heart of care delivered and the front face to every organization. Respicio, et al. (2018) reminds us that we need to consider not only demand for nursing, but the need to safeguard health and wellbeing of the nurses also, beyond guidelines and legislation in force. In staffing for nursing, units based on acuity and pts needs and in acute care settings we need to be prepared for admissions and the shift is not predicted. Compared to the skilled unit, that is not a place that has unexpected admissions so the staffing should be easier to do. Staffing ratio depends on several conditions and should be accounted for when looking at scheduling.
McGarry, et. al (2021) reports that due to Medicare’s payment model, they have seen a reduction in therapy staff for skilled nursing facilities. This effects the nursing aspect because now more nursing staff is needed in nursing to cover care that patients need if therapy is reduced. In addition, there are guidelines that are made and following with minimal number of hours per patient that is totaled dependent on where you work. skilled approach is team based. In medical-surgical units have the flexibility to use float pull nurses which are not available for skilled nursing. Hours reflect are on a 24-hour bases of census and ratio.
Key differences are when in acute care hospitals nurses can be pulled from other departments while in skilled nursing, the nurses get stretched or management comes in to assist. Since the population need is different, they need to shop according to that and use their own staffing model that will work best for themselves.
