Curricular Considerations: Roles and responsibilities of health care providers for health information
documentation.
TASKS: Investigate the responsibilities of different healthcare providers in different healthcare settings for
documentation requirements, and the implications for noncompliance with documentation standards, according
to regulatory and accrediting agencies.
Choose one of the following settings:
Acute Care Hospital
Mental Health Clinic
Substance Abuse Hospital
Rehabilitation Facility
Determine the different providers responsible for documenting the health record in the healthcare setting of
your choice;
Indicate which documents the various providers are responsible for completing;
Explain the implications for the providers and facility for noncompliance with documentation standards.
In a 2-3 page essay, indicate the setting you chose to research, and detail findings pertaining to documentation
standards, responsibilities of the different providers, and the various documents found in the health record in
this particular setting. The consequences for noncompliance with documentation standards, for both the
providers and the facility, must also be included. The conclusion should contain your understanding of the
importance of strict adherence to documentation requirements by providers across the continuum of
healthcare. The essay must be double-spaced, in Times New Roman, size 12 font, using correct sentence
structure, proper grammar, spelling, punctuation and formatting. A works cited page must be included, in MLA
format and supporting documentation of your research must be attached for final submission.
