It is well generally well accepted that, for medical practice to be
considered “ethical,” it must respect all four of the basic ethical
principles of autonomy, justice, beneficence, and non-maleficence.
Formulating an approach to operationalizing the health care reform
provisions of the Affordable Care Act, however, the American College of
Emergency Physicians (ACEP) moves beyond these basic principles. For
example, addressing unsustainable health care costs, ACEP identifies
distributive justice as the primary ethical principle supporting the
imperative to achieve cost containment.
Striking a workable balance between providing value in health care and
bringing down costs poses a fundamental ethical challenge for health
reform. As others also have cautioned, ACEP warns that, unless we
contain health care costs that are already too high, and continue to rise,
it will become increasingly difficult to provide care in a reformed health
care system.
In addition to cost containment, ACEP cites public health needs, access,
emergency department crowding, and end-of-life care as ethical issues
inextricably linked to true health reform. Please select one of the ethical

what data-driven strategies would you recommend to resolve the ethical

The Robert Wood Johnson Foundation estimated that health disparities
cost the U.S. more than $60 billion in 2009. Health care organizations
across the country have recognized health disparities as a pressing issue,
and many have taken action.
One initiative that has gained some traction is the collection of patients’
race, ethnicity, and language (REL) data. Collecting this data reportedly
helps health care providers pinpoint where they have disparities in care
and creates accountability in addressing them. The benefits in terms of
cost containment, quality of care, and improved patient outcomes are
most evident.
However, the collection of REL data also presents an ethical dilemma.
What ethical issue(s) does this kind of data collection address? What
ethical principle(s) does it uphold? Has your analysis led to the
conclusion that REL data collection invokes an “ethical dilemma?” Why,
or why not?
